The
GBMS leadership team conducts ongoing needs assessments
to initiate and improve services to meet the ever-changing
needs of our service communities. Our special programs
are designed to build healthy communities as we have assessed
a need for these services in southern Maryland.
Women,
Infants and Children (WIC)
Our
new WIC Program is a supplemental nutrition program
for women, infants and children under 5. The clinic
provides healthy supplemental foods and nutrition counseling
to pregnant women, new mothers, infants and children
under age five. The nationally recognized WIC program
has an extraordinary 25 year record of preventing children's
health problems and improving their long-term health,
growth and development. For more information on WIC
services and eligibility, contact Jackie Melton, WIC
Coordinator, 301-324-1873.
Van
Transportation
Realizing that transportation can be a barrier to obtaining
healthcare services, we provide free transportation
services to persons in rural Prince George’s and
Charles Counties. Our van service is guaranteed to get
patients to their appointments on time and return patients
to their residence afterwards. For more information
on van transportation service, contact Margaret Johnson,
Van Transportation Coordinator, 301-888-1444.
Homeless
Education and Outreach
Since the program’s inception in 2002, we have
increased accessibility to healthcare for over 120 persons
in homeless shelters in Prince George’s County.
Through our weekly outreach sessions, we have presented
health education to nearly 300 homeless residents. From
disseminating information on disease prevention, to
conducting in-services on developing effective resumes,
our outreach staff works avidly to meet the holistic
needs of the homeless population. For more information
on outreach efforts to the homeless, contact Martha Ramirez, Health Educator, 301-599-0460 x1114.
HIV
Care
As a Ryan White Title grant recipient since 1991, our
system has provided healthcare services to 237 persons
with HIV. Through training and experience, our staff
and providers are adept in meeting the unique clinical
needs of persons with HIV. Services rendered include
and are not limited to: primary care, case management,
dental care, emergency drug assistance, mental health
counseling and specialty referrals. Read our brochure on HIV Care (pdf) . For more information
on HIV services, contact Rachel E. Smith, MSc, Director
of Program Management, (301) 599-6379.
Diabetes
Care
As diabetes continues to affect the lives of thousands
of Marylanders, GBMS is prepared to provide the intervention
needed to either prevent diabetes or assist persons
in effectively managing their conditions. To supplement
care provided by our providers, GBMS conducts group
sessions for persons with diabetes. The goal for the
sessions is to establish a support network for patients
through information exchange and goal-setting. For more
information on diabetes programs, contact Debbie Apperson,
Nurse Practitioner, 301-324-1500, 301-599-0460, x1107. |
