We are currently accepting new patients at our Capitol Heights, Brandywine, and Leonardtown locations. We are accepting new pediatric patients at our Oxon Hill location.

  If you are a seeking an appointment it is important that you fill out the forms under New Patient Forms. Please print and complete these forms and bring them with you to your first appointment.

  Greater Baden also accepts CARE patients from other clinical providers. To enroll a client in the Greater Baden CARE program for medical care, or dental care, please use the forms located by clicking on for Referring Professionals. This will help make possible a smooth transition to Greater Baden Services.
